Medical Social Worker – Exceptional Family Member Program

Place of Performance: The work to be performed under this contract will be at David Grant Medical Center (DGMC), Travis AFB, CA. HCWs shall receive notification two weeks prior to the reassignment to another location within a 40-mile commuting radius of their assigned MTF. 101 Bodin Cir Ste 777, Travis AFB, CA 94535

Degree/Education (5.2.4): See Exhibit 5 of the Defense Health Agency’s Medical Q-coded Series (MQS) Indefinite Delivery/Indefinite Quantity (IDIQ) contract, hereafter referred to as “Exhibit 5”

  • Degree: Master's degree in Social Work (MSW).
  • Education: Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
  • Licensure: Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker/MSW
  • Certification: Certifications in addition to Basic Life Support (5.2.3): None
  • Experience: Must have a minimum of two (2) years full-time experience in care coordination, discharge planning or case management in medical setting.

HCW Duties (5.1 Task 1): In addition to Exhibit 5, the duties for the HCW are:

  • Knowledge. Must have the knowledge and skills to effectively apply Special Needs Coordinator functions. Must be at least moderately computer literate; must be familiar with standard MS Office, Adobe PDF, general database familiarity and other good general computer/electronic office equipment functions/use.
  • Mastery of theories, principles, and methodologies underlying psychosocial practice.

Knowledge and understanding of developmental growth; dynamics of human behavior, family, and other social systems; and impact of illness and disability on social functioning.

  • Demonstrated abilities in program planning, implementation, and evaluation, and in conducting individual, family, group, and community assessments.
  • Strong capability to work with service members and their families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems.

· Working knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act (HIPAA), and accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC).

· Must demonstrate ability to communicate effectively both orally and written.

Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and potentially agitated.

  • Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations.

· Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.

HCW Duties (5.1 Task 1): In addition to Exhibit 5, the duties for the HCW are as follows:

  • Establishes and maintains procedures to identify sponsors whose family members have special medical and/or educational needs in a timely manner.
    • Conducts assessments of family members of Regular Air Force (RegAF) sponsors to determine if special medical and/or education conditions exist that require Exceptional Family Member Program (EFMP) enrollment.
        • Ensures all RegAF sponsors known to the MTF as having family members with special

needs according to DoD criteria are identified to the Air Force Personnel Center (AFPC) (via the local Military Personnel Section (MPS) and/or Commander Support Staff (CSS), where applicable) for issuance of an assignment limitation code (ALC) “Q”, regardless of the location of family members or the source of family health care.

  • Ensures all Army, Navy, and Marine Corps sponsors of family members with special needs are referred to their Services’ EFMP Point of Contact (POC) for enrollment and

relocation support IAW Service-specific guidance.

  • Ensures every RegAF sponsor with one or more family members with special needs that is

is assigned to the installation has an SN file maintained at the MTF and a case established in the AF-provided special needs data system (Q-base).

  • Coordinates, with AFPC (and the local MPS and/or CSS, where applicable) all assignment actions for all Q-coded AF sponsors assigned to the installation, or whose personnel functions are handled at the installation, regardless of location of family members.
  • The SNC is involved in the Family Medicine Relocation Clearance (FMRC) process as outlined in AFI 40-701, Medical Support to Family Member Relocation and Exceptional Family Member Program.
  • Ensures Q-coded sponsors assigned to the installation are contacted annually to update their EFMP status, and provide any updates needed in contact information, family composition, or family member conditions; verification of need for continued enrollment as well as pending Permanent Change of Station (PCS) should also be discussed.
  • Leads the Exceptional Family Member quarterly case reviews to discuss newly identified families, complex or unmet needs and determine appropriate resources necessary for families.
  • Recognize and remain up to date on limitations imposed by MTF capabilities, and local national and other DoD community resources.
  • Conducts assessments of family members of RegAF sponsors to determine if special

medical and/or education conditions exist that require enrollment.

  • Assessment: The Special Need Coordinator (SNC) must have the ability to identify patients and families with special medical and educational needs in a timely manner.
  • The SNC uses a multi- disciplinary and collaborative approach with other key service providers, such as installation youth and childcare facilities, and officer and enlisted spouses’ groups, to ensure effective outreach and identification of special needs.
  • Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care.
  • Facilitation: Care coordination and communication among all involved parties is key to provision of care for individuals with special medical needs.
  • Advocacy: Support for the patient and family/caregivers to ensure proper education of the patient and family and appropriate, timely care coordination is received.
  • Verifies eligibility of beneficiaries using Defense Enrollment Eligibility System (DEERS). National Agency Check with Local Agency Checks and Credit Check (NACLC) required.
  • Educates appropriate medical staff on aspects of the EFMP program annually and may be asked to present this verbally.
  • The social worker oversees and manages the installation Exceptional Family Member Program-Medical (EFMP-M) IAW DoD and AF policy and any subsequent implementing guidance.
  • Ensures all active-duty sponsors known to the AFPC, and local MPS and/or CSS where applicable, having family members with special needs are identified in AF medical special needs data management systems. Provides oversight for base-level data entry in AF-provided data management systems used in the management of EFMP-M.
  • Provides training to installation personnel and medical staff; and consultation as needed to support the implementation of EFMP-M base-wide.
  • Integrally involved in the FMRC process. Ensures all FMRC requirements are implemented by all EFMP-M staff. Coordinates the enrollment process and travel screening for all active-duty family members’ and travel screening upon request to the families of DoD civilian sponsored assignments going to an overseas base.
  • Collaborates with the Integrated Delivery System (IDS) and other interagency forums that provide family medical information and referrals to base and civilian agencies.
  • Ensures EFMP families are referred to the Airman and Family Readiness Center for additional community assistance as needed. Maintains a cooperative working relationship with the base MPS, CSS, and with AFPC. Actively supports the integration of EFMP-M, Exceptional Family Member Program – Family Support (EFMP-FS) and Exceptional Family Member Program – Assignments (EFMP-A) services at the installation.
  • Patient specific assessments and clinical plans of action will be located in Armed Forces Health Longitudinal Technology Application AHLTA and the Special Needs record. Meeting minutes will not contain patient identifying information.
  • Coordinates and participates in designated facility meetings.
  • Participates in the clinic orientation and training of other staff of the Special Needs Program.
  • May serve on committees, work groups, and task forces at the facility.

  • Must maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional social work, Health Services Inspection (HSI); and other applicable DoD and service specific guidance’s and policies.

Must also comply with Exhibit 5 Core Duties:

  • Core Duties:
  • Perform a full range of pharmacist procedures.
  • Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment

capabilities of the facility.

  • Counsel patients regarding appropriate use of medications.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or

overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above listed items.

  • Compound medications as required.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled

pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all

scheduled controlled substances according to applicable regulations.

Scheduling: The schedule or scheduling process is as follows: The schedule is maintained by clinic personnel based on demands of the clinic and appointments booked that day. The specific schedule for each two-week period will be provided one month in advance. At the mutual agreement of the Contractor and the Government, alternative schedules may be implemented such as a compressed work schedule. Any changes in the schedule shall be coordinated between the HCW and the Government. Any planned absences of the HCW shall be communicated to the Government supervisor in advance of the absence to ensure that clinical operations are not adversely affected. The Government reserves the right to change shift hours with a five-day notice to accommodate patient care needs and mission requirements.

Closures: During a planned closure of the facility due to training, holiday, or unplanned closure due to unusual and compelling circumstances (e.g., natural disasters, military emergencies, severe weather), the Contractor will: ? be compensated only for the actual hours the HCW provided services ? be compensated for the unplanned closure (personal services only)

Unplanned Closures: Other than the Federal Recognized Holidays as described in paragraph 1.5.6, the following are considered training days or days where the HCW will not be required to work on this TO:

Mission Essential: The following labor categories are designated as mission essential: Medical Social Worker is NOT Mission Essential.

Computer Skill Competency: Each HCW shall demonstrate competency as required in the contract and as follows: Medical Social Worker – Yes

MTF Training: Additional training requirements are as follows: Complete MTF Newcomers Orientation and annual training requirements to include, but not limited to: Safety Storm Training, Health Insurance Portability and Accountability Act (HIPAA), Trusted Care Training, Interventional Violence Prevention Training (IVPT), Sexual Assault Prevention and Response (SAPR), Suicide Prevention, Computer Based Training (CBT) from sites including MEDLEARN, Joint Knowledge Online (JKO) and Relias. Contractors must complete CBTs once they are allowed on base and have computer access.

MTF Standards: Specific policies, procedures, and instructions/regulations for the place of performance are as follows: HCWs will comply with all relevant DoDI, AFIs, AFMANs, MDGIs, policy letters and HHQ directives; including, but not limited to, DHA policies and regulations, DoDMs, DoDDs and other pertinent guidance. Additional job specific requirements are listed in the MDGI Operating Instructions, provided upon request.

Competency-Based HCW Interviews: Prior to HCW placement, the Government ?may ? will not contact the HCW to determine if the HCW meets the requirements set in the contract and/or task order. A competency based HCW Interview may be conducted to determine if the candidate is qualified (i.e., ready) desires the job (i.e., willing) and capable (i.e., able) to work at the place of performance.

HEALTHCARE REQUIREMENTS

1.6.20 HCW Health Requirements: The Contractor ? shall ? shall not provide documentation certifying health requirements such as immunizations, annual vaccinations, medical testing (i.e., tuberculosis, N95 particulate respirator duckbill mask fitting) and physical examination when required at the time of initial placement.

Occupational Health: The HCW ?shall ?shall not be required to obtain documentation of required physical testing or a report of a physical examination.

Tuberculosis Screening: The additional immunization/screening requirements for the risk of exposure to tuberculosis (TB) are as follows:

  • HCWs with a baseline positive or newly positive test result for M. tuberculosis infection (i.e., TST or BAMT) or documentation of treatment for LTBI or TB disease should receive one chest radiograph result to exclude TB disease or an interpretable copy within the past 12 months.
  • 1. Two-step tuberculin skin test (TST) for M. tuberculosis (BAMT) that was performed within the previous 12 months

OR

  • 2. An approved blood assays for M. tuberculosis (BAMT) that was performed within the previous 12 months.

GOVERNMENT SECURITY PROCESS

5.2.9.1 HCWs shall be subject to the following additional security investigative processes, to include appointments with Security Managers: To initiate security review, the contractor will submit a completed and accurate Common Access Card (CAC) Vetting Form to the MTF Security Manager. Information will include, but is not limited to, the HCW full name, other names used (maiden), date of birth, social security number, email address, physical address, home telephone and mobile phone numbers, place of birth (city, state, country), citizenship, prior military end date, reserve status and branch, prior government end date.

5.2.9.3.3. The contractor shall advise their HCWs that favorable fingerprints with an open investigation, verified through Defense Information System for Security (DISS) Web Application is needed as a condition of employment under this TO. The contractor understands that while the MTF may allow contracted HCWs temporary or interim access to Government systems in a non-sensitive position, pending the outcome of the T1, this contract requires the immediate removal of a contracted HCW from the position if their investigation return with an “unfavorable” adjudication.

5.2.9.4 HCWs shall be subject to the following security processes for fingerprints: Fingerprints will be taken during the first week of assignment to the MTF at the Travis AFB Information Protection Office. Appointment date and time will be provided on the first day of assignment.

5.2.9.5The HCW shall complete either a ?SF-85 or an ?SF-86 Questionnaire for National Security Positions (or equivalent OPM investigative product).
