Job Description

Job Summary: Provides psychosocial assessments, diagnosis and treatment to patients in their place of residence as prescribed by a physician, to assist the patient/family develop effective coping skills in response to the patient’s diagnosis, treatment and/or life situation.

Key Responsibilities:

• Perform accurate and comprehensive assessments.
• Obtain physician orders for certification and recertification of home medical social work services.
• Communicate changes in patients’ clinical status to healthcare team members and HHA office staff.
• Coordinate care planning and delivery with the physician, patient/family, and other healthcare team members.
• Educate patient/family and other staff on appropriate medical social work modalities and interventions.
• Deliver, modify, or discontinue medical social work interventions based on ongoing assessments.
• Ensure patient safety and reduce the likelihood of medical/health care errors.
• Identify and address risks in the patient’s environment.
• Perform transfer and discharge planning functions.
• Participate in the organization’s information management program.
• Comply with HHA policies and adhere to all applicable laws, regulations, and standards.

Regulatory Requirements:

• Current valid State clinical social worker license.
• Master’s or doctoral degree from an accredited school of social work.
• One year of post-master’s experience in a clinical setting.
• Prior home health experience preferred.
• Valid state driver’s license and reliable automobile.
• Proof of current automobile insurance.
• Current health certificate/physical examination and TB testing.
• Current BCLS certification.
• Criminal background check.
• Current liability insurance.

Language Skills:

• Effective communication in English, both verbally and in writing.
• Additional languages preferred.

Skills:

• Understanding of regulations/standards applicable to home healthcare.
• Knowledge of general medical social work modalities/interventions and scope of practice within Arizona.
