Position Summary: Provides professional delivery of nursing services in the Outpatient/Ambulatory Clinic. Answer patient telephone calls requiring use of the nursing process in collaboration with licensed practitioner(s) to: address patient needs, medication refills, appointment scheduling and miscellaneous duties as assigned which may include: Referral Management, Medication Prior Authorization and Annual Wellness Calls.
Excellence in Practice: - Participates in nursing care, health maintenance, health teaching, development and modification of the patient’s plan of care in collaboration with licensed practitioners and other licensed care team members while working within scope of licensure.
- Assists, contributes and participates in the nursing process by performing a focused assessment on individual patient status and needs over the telephone to collect, communicate, document data, making appropriate referral or appropriate nursing advice within scope of licensure .
- Is knowledgeable in the assessment, treatment and care of specific populations such as neonatal, pediatric, adolescent, adult, geriatrics and developmentally disables as appropriate for level of licensure and responsibilities .
- Recognizes changes to patients health status, implementing nursing interventions, prescribed medical regimens, and established standard of care as ordered by a licensed practitioner in appropriate timeframe while working within scope of licensure. Collaborates changes and intervention responses with licensed health care team.
- Communicates relevant patient information to licensed practitioner and health care team time appropriately.
- Documentation of relevant patient information and care is completed accurately and time appropriately.
- Demonstrates ability to prioritize delivery of ambulatory services.
- Assists licensed practitioner as requested, required and appropriate. Actively participates in direct delivery of safe patient care according to licensed practitioner’s instructions and orders, in conformance with recognized techniques, procedures and standard of care.
- Safely administers medications and IV therapy routinely and on an emergent basis as order by licensed practitioner. Follows 5 Rights of Medication. Demonstrates knowledge of medication purpose, dosage, calculation of dosage and possible adverse reactions. Provides appropriate patient education prior to administration of medications, immunizations and allergens. Accurately documents administered medications, immunizations, allergens and procedures performed.
- Demonstrates ability to utilize EHR/Allscripts appropriately and confidentially.
- Demonstrates quality performance in skills. Seeks guidance and direction. Acts within scope of licensure.
Essential Job Requirements:
- Education: RN / LPN License
- Experience: On-site training
- License Requirements: ND and/or SD RN / LPN Licensure.
- Basis Life Support and ACLS certification if required by the clinic manager.
